Let users add multiple choice configurable fields to tasks

When users are adding additional fields to task types, allow the ability to add a multiple-choice field (in addition to the current write-in field). Example, for a photo assignment task: Is the shoot time flexible? Yes / No.

Permissions service changes should be id'ed and timestamped

Roles/squads should show the last person to revise them and when. A Slack integration would be even better. WashPost has had two problems this week with permisisons changes that we can't track.
